1) Grade Verification/Concussion Protocol/Medical Release
2) Most recent Grade Report Card
3) Tehama County Fairgrounds Release of Liability
These forms MUST be uploaded before April 19th to avoid a $25 fee. After may 1st the contestant will be removed from the rodeo
